Use of Theros® FMS Dilators for Hemorrhoids

Here we describe the use of FMS dilators for hemorrhoids.

Repositioning

The primary purpose of using glass dilators for hemorrhoids is repositioning. Repositioning means gently returning the hemorrhoid to its original position within the hemorrhoidal cushion from which it has protruded. Temperature adjustment of the dilators is particularly helpful here – a clear advantage of FMS dilators. For inflamed hemorrhoids, cooling is the optimal measure to reduce swelling. As the hemorrhoid becomes smaller, repositioning becomes easier. Cooling may also help relieve pain.

The optimal size of a dilator for repositioning depends on many factors:

• the severity of the symptoms,
• individual anatomy,
• rectal flexibility and capacity.

For this reason, it is highly advisable to have several dilator sizes available – in other words, a dilator set – so that you can choose the most suitable size according to your condition on a particular day.

The treatment is performed while lying comfortably on your side.

1. Adjust the temperature of the dilator, either by placing it briefly in cold water or (for a stronger and more lasting effect) in a refrigerator, as described below under “Dilators as Cooling Devices”.
2. The dilator is dried if necessary, lubricated and inserted slowly using gentle pressure. During insertion, the hemorrhoid is repositioned (pressed back into the hemorrhoidal tissue). At the same time, the cooling effect helps constrict the opening through which it protruded.
3. The dilator should remain in place for at least fifteen minutes to make repositioning more sustainable.
4. Afterwards, the dilator is removed carefully. In some cases, the hemorrhoid disappears completely after the first application. More commonly, treatment must be repeated over several days until the hemorrhoid remains permanently within the hemorrhoidal cushion.

Dilators as Cooling Devices

The special material of an FMS dilator already provides a pleasantly cooling effect at room temperature. This effect can be enhanced further by placing the dilator in cold water after it has warmed up during use. For genuine cryotherapy (cold therapy), FMS dilators can be filled with water-soaked kitchen paper. The tissue paper reliably retains the water inside the dilator. Once prepared in this way, the dilator can be placed in the refrigerator for several hours until it is thoroughly cooled.

Strengthening, Aftercare and Prevention

Once symptoms have subsided, regular massage with FMS dilators is an ideal form of aftercare because it strengthens the tissue and effectively helps prevent the recurrence of hemorrhoids. The massage effect is created by the back-and-forth movement of the dilator with its elliptical head – a feature unique to FMS dilators. Borosilicate glass, with the smoothest surface of all materials, reliably prevents irritation of the rectal mucosa and the anal sphincter.

An additional advantage of FMS dilators is the possibility of training the alternation between relaxation and contraction in synchrony with the movement of the dilator:

During insertion, focus on consciously relaxing the sphincter muscle. Afterwards, train the opposite movement: during careful withdrawal, gently resist by attempting to hold the dilator in place using your pelvic floor and sphincter muscles. Repeat this several times and regard both phases of the exercise – relaxation and contraction – as equally important. In this way, you significantly strengthen the entire pelvic floor as well as the sphincter muscles of the bladder and anus.

Choosing the Right Size(s)

The exceptional effectiveness and ease of use of FMS dilators result from the ability to adapt the size precisely to your individual needs. People differ greatly in rectal capacity and flexibility. Gender, age, severity of symptoms and previous experience with rectal insertion or anal eroticism all contribute to substantial differences.
