It depends on the condition or problem you're dealing with. In some cases, using a single dilator in a specific size may be sufficient—for example, with haemorrhoids. However, even in such cases, having a small range of about three sizes is generally more advisable, depending on your (daily) condition and the severity of symptoms. A dilator that is too small may have little to no effect, while one that is too large may not be insertable at all.For all conditions in which the body needs to be (re)opened gradually, muscle tension needs to be released, or trauma needs to be processed, a larger set of at least four dilators is essential. This applies to vaginismus, lichen sclerosus, scar treatment, gender reassignment surgery, radiation therapy effects, anal cramping, anal fissures, menopause-related symptoms, sex after long abstinence, etc. A gradual increase in size is crucial.

Under no circumstances should insertion—or the gentle in-and-out movement, which is key to effectiveness in some applications—cause pain when using the smallest (initial) size of the treatment.

However, all problems in which the body needs to be gradually (re)opened, cramps relieved and trauma treated absolutely require a larger set of at least four. This applies to vaginismus, lichen sclerosus, scar treatment, genital surgery, the effects of radiation, anal cramps, anal fissures, menopausal symptoms, sex after a long period of abstinence, etc. in order to be able to achieve a gradual increase. Under no circumstances should the insertion or the slow back and forth movement - which is the actual effect in some applications - cause pain with the smallest size (initial size of the treatment).

Although all FMS dilators have an elliptical head and a straight shaft, a closer look reveals that the shapes are subtly different: these different silhouettes result from the specific ratio between the head and shaft diameters (see detailed specifications for each size). The selectable size of a dilator always refers to the maximum diameter of the elliptical head. The silhouette is fixed for each size and cannot be chosen separately.

Each silhouette has a slightly different effect:

  • The smaller the difference between head and shaft diameter, the greater the overall stretching effect, because the relatively thick shaft occupies more volume in the body than a slimmer one.
  • The larger the difference between head and shaft, the stronger the massage effect during movement, as the tissue responds more intensely to the contrast—creating a pulsing sensation.

There is no such thing as a “perfect” silhouette—each one provides a unique kind of stimulation. Within a set, these different silhouettes add even more variation to the sensory experience.

In the detailed product descriptions for sizes 14 to 19 mm, you’ll find all seven silhouettes in sequence: you can clearly see how the head size increases while the shaft remains the same. These silhouette differences also exist in other size ranges, always within atherapeutically meaningful variation - from a minimum of 2 mm to a maximum of 8 mm difference. This variation is also proportional to the diameter: the larger the dilator, the greater the possible difference.

Every person is different, and every condition has its own form and severity. That’s why our products allow for a highly individual selection. The key to finding the right size for a set is to become familiar with your own body.

To help determine the right size, you can use common objects such as different sizes of tampons, pens, handles, or candles, as well as conventional erotic items. Wrapped safely in a condom, these can be tested gently and carefully.

Don’t be shy! This approach might feel unfamiliar or strange at first, but taking this initiative and exploring your body is the first important step toward healing.

The smallest increments of 1 to 2 millimeters (D14 to D20 in 1 mm steps, D20 to D60 in 2 mm steps) are primarily intended for the initial treatment of sensitive or acute trauma and wound care—such as post-surgical injuries, birth-related trauma, radiation damage, inflamed haemorrhoids, etc.—where an especially gentle approach is required.

However, these fine gradations also allow for a generally more careful and faster progression, sometimes even enabling an increase of several sizes within a single session.

With increments of 2 to 4 millimeters, longer usage times per size are necessary before moving up. Larger increments within a set are not recommended.

That’s no problem. Once you receive your set of dilators, start by unpacking the largest dilator and try it gently—most likely, it will still be far too large. Then try the next smaller size, and continue until you find one that you can insert easily. That’s your ideal starting size.

Keep in mind that for certain applications (e.g. vaginismus,lichen sclerosus, scar treatment, etc.), the size should be small enough not only to allow insertion, but also to permit comfortableback-and-forth movement for gentle massage.

Any smaller dilators that you don’t need can be exchanged. You can return them as long as the seals remain intact—which is the case if you’ve followed this approach.
